A person who is said to have a monkey on his or her back most likely appears to be struggling with an overwhelming and troubling burden. A persistent problem might be described as a monkey on one’s back.
Most commonly, however, this common phrasing is used to refer to drug addiction. If a person is described as having a monkey on his or her back, that individual is most likely struggling with a dangerous chemical habit. Occasionally, the "monkey on one’s back" expression is used to describe other addictions as well, such as alcoholic addiction, addiction to tobacco or acquired dependencies on other substances.
The monkey metaphor seems particularly fitting for such instances, where dangerous addictions may have begun with recreational experimentation. Monkeys, after all, may seem entertaining and even harmless at first. But, wild creatures that they may be, monkeys can become threatening and even perilous, if they are allowed to get out of control.
