Okay, so I took a urine drug screen today, it was being sent to a lab with a chain of custody form and a sealed cup etc etc. I have a legal prescription for Adderall XR, but I have taken some phentermine for about the last two weeks that I do NOT have a prescription for. Should I be worried?? Since Adderall is an amphetamine, will the phentermine be the same or can they tell the difference?

It really depends on which testing method they’re using. It’s my impression that they first run an initial screen with immunoassay methods like ELISA. Apparently, phentermine will cross-react with the amphetamine antibody, but not methamphetamine.

My guess is that if they flag amphetamine and you have a legitimate script for adderall (amphetamine), they might not bother to look into it further. If on the other hand they go to confirm with more sophisticated and specific methods like GC Mass Spec, they’re not going to find the proper amphetamine metabolite peaks, and things could get dodgy. A lot of this might come down to what sort of employer/agency is testing you. If it’s a basic pre-employment screen from some retail chain, they’ll probably want to keep it simple and cheap.

If it’s a thing for parole, or sports or some sensitive government job or heavily regulated transportation thing, they’re likely to be more thorough. In general, they seem to use confirmation tests as a safeguard. If the first run shows generic “amphetamine” and you insist its a false positive from, say, cold meds, they’ll look to confirm and nail you in a lie in confirmation. Common sense says if they flag amphetamine and you admit to it and have a legitimate medical reason for it, that will be the end of it. Another thing to keep in mind is that phentermine is considered to have lower abuse potential and is less tightly regulated. When people think of drug abusers, they tend to think of people who go for “heavier” drugs than they’re authorized to have, not lesser ones. All that said, when you’re subject to workplace drug testing and you play games with it, you’re gambling with your livelihood.
